INITIAL VISIT
A link will be sent to your email where you can create a secure patient account through Patientally.com. After your account is created, you can fill out your "new patient" packet, view pertinent documents, request appointments, view lab results, and send a message to your provider. If you prefer to bypass this method, you may request all documents to be sent to your email to complete and fax or email them back to us before your appointment time.
Please plan to arrive 15 minutes before your scheduled appointment if meeting in person. If you have a telemedicine appointment, please log in at your scheduled time. Your provider will be waiting for you.

BEFORE YOUR APPOINTMENT BEGINS Please have the following:

A complete list of current prescriptions, medical and surgical history

At Monarch Mental Health & Wellness Services, we try to make it as easy as possible to set up an appointment in the shortest time possible.
We do ask that you take your appointment time seriously.
NO SHOW POLICY

A new patient appointment marked as a "no show" for any reason may limit future appointments.

Please arrive 15 minutes prior to your appointment for in-person appointments and log in at your specified time for telemedicine appointments. A $25 "no show" fee may be required to reschedule your appointment.
If, for any reason, you cannot keep your appointment please notify us at least 24 hours before the scheduled appointment time.
CANCELLATION POLICY
If you need to cancel your appointment, we ask that you give at least 24 hours notice.
Appointments that are cancelled less than 24 hours or for "no show" appointments, you will be assessed a $25.00 cancellation fee. This must be paid prior to your next visit.
MEDICAL RECORDS
A signed consent is required if you wish to have copies of your records sent to another doctor or organization.
You will need to include the specific dates of service, medical information requested and the reason for this request.
PRESCRIPTION RENEWALS
Prescription renewals are most efficiently handled through our practice's messaging system which is available on this website.
Please try to anticipate your need for prescription refills by notifying the office at least 72 hours in advance or by advising your physician of your needs during regularly scheduled office visits.

If you have not been seen in this practice within the past 3 months, it is our policy that you make an appointment to renew your medication.
UPDATING YOUR INFORMATION
Patients are responsible for providing accurate, up-to-date information. You can update your insurance, personal and medical information from within "Health Record" in this website.
As you may have several physicians, this information can change without the treating doctor being aware of these changes. We ask that you login prior to any scheduled appointment to keep this information current.
